Despite significant advances in medical science, Tuberculosis (TB) remains a global health challenge. Early and accurate diagnosis is critical to control its spread and improve patient outcomes. This article provides a comprehensive review of modern TB diagnostic techniques.
PCR-based tests, such as the Xpert MTB/RIF assay, have revolutionized TB diagnostics. They offer rapid, sensitive, and specific detection of Mycobacterium tuberculosis and rifampicin resistance. The WHO recommends these tests as initial diagnostics in adults suspected of having MDR-TB or HIV-associated TB.
Interferon-gamma release assays (IGRAs) and tuberculin skin tests (TST) are the mainstay of latent TB diagnostics. IGRAs, which measure the immune response to TB-specific antigens, are more specific than TST and are not affected by prior BCG vaccination. However, neither test can distinguish between latent infection and active disease.
Chest radiography remains a vital tool in TB diagnosis, especially in resource-limited settings. It can provide rapid results, but its sensitivity and specificity are variable. Computed tomography (CT) and magnetic resonance imaging (MRI) offer better resolution and can detect extrapulmonary TB, but their high cost and limited availability restrict their use.
Culture remains the gold standard for TB diagnosis, providing definitive evidence of infection. It allows drug susceptibility testing, but it is slow and requires sophisticated laboratories. Newer techniques, such as line probe assays, offer faster results and simultaneous detection of drug resistance.
In conclusion, an array of diagnostic tools is available for TB, each with its strengths and limitations. Rapid molecular tests have improved the speed and accuracy of diagnosis, but culture remains the gold standard. The choice of diagnostic approach should be tailored to the patient's clinical presentation and the resources available.
