This review examines the scientific rationale, epidemiological trends, pathophysiology, clinical implications, and management strategies for fertility preservation in women of AMA. Emphasis is placed on evidence-based counseling, recent advances, emerging techniques, and guideline-driven recommendations to optimize reproductive outcomes for this growing patient population.

Introduction

With sociocultural shifts and greater female participation in higher education and the workforce, the trend toward delayed parenthood has become pronounced worldwide. Advanced maternal age, typically defined as 35 years and older, presents distinct reproductive challenges, including reduced fertility, increased obstetric risks, and heightened perinatal complications. Fertility preservation (FP) counseling now forms an integral aspect of reproductive medicine, providing women with options to mitigate age-related fertility decline. This article synthesizes current evidence and clinical guidance to equip healthcare professionals with a comprehensive approach to FP counseling in AMA.

Epidemiology / Disease Burden

The average age of first-time motherhood has risen in many developed countries, with significant increases in women having children after age 35 or even 40. Epidemiological data from Europe and North America indicate that more than 20% of live births are to women of AMA. The cumulative live birth rate declines sharply after age 35, driven by both quantitative and qualitative losses in ovarian function. Women of advanced maternal age often present with a history of subfertility or infertility, irregular menstrual cycles, diminished ovarian reserve, and increased risk of pregnancy complications such as gestational diabetes, hypertensive disorders, and chromosomal abnormalities in offspring. The clinical course is influenced by the degree of ovarian aging, coexisting comorbidities, and prior reproductive history, underscoring the need for individualized assessment and management planning.

Diagnosis

Assessment of ovarian reserve is central to fertility counseling in AMA. Serum anti-Müllerian hormone (AMH) levels, antral follicle count (AFC) via transvaginal ultrasound, and basal follicle-stimulating hormone (FSH) measurements are commonly used markers. Advanced techniques such as ovarian reserve genetic testing and dynamic ovarian stimulation protocols provide additional prognostic information. Comprehensive reproductive evaluation also includes assessment for tubal patency, uterine pathology, and partner factors.

Treatment & Management

Fertility preservation strategies for women with AMA include oocyte cryopreservation, embryo cryopreservation, and, less commonly, ovarian tissue cryopreservation. Oocyte vitrification is now well-established, with improved survival and pregnancy rates, especially when performed before age 35. Embryo cryopreservation is considered in patients with partners or those using donor sperm. Timely referral to reproductive endocrinology specialists is vital for optimal counseling and intervention. Adjunctive measures such as lifestyle modification, weight optimization, smoking cessation, and preconception health optimization further enhance reproductive outcomes.

Recent Advances / Emerging Therapies

Technological improvements in oocyte vitrification, preimplantation genetic testing for aneuploidy (PGT-A), and ovarian rejuvenation therapies (e.g., platelet-rich plasma, stem cell therapy) are expanding the therapeutic landscape for AMA. Advances in in vitro maturation (IVM) and artificial intelligence-based embryo selection hold promise for increasing success rates in this population. Ongoing research into ovarian aging mechanisms and novel biomarkers may enable earlier intervention and more precise risk stratification.

Guideline Recommendations

International guidelines from the American Society for Reproductive Medicine (ASRM), European Society of Human Reproduction and Embryology (ESHRE), and the American College of Obstetricians and Gynecologists (ACOG) recommend proactive fertility counseling for all women considering delayed childbearing. Counseling should cover age-related fertility decline, success rates of FP techniques, risks of delayed parenthood, and the importance of early intervention. Shared decision-making and individualized care, based on patient preferences and clinical context, are emphasized throughout the counseling process.

Conclusion

Fertility preservation counseling is a critical component of reproductive healthcare for women of advanced maternal age. Clinicians must remain abreast of evolving evidence, technological advances, and guideline recommendations to provide comprehensive, patient-centered counseling. Early identification of at-risk women, timely intervention, and ongoing support are essential to optimizing reproductive outcomes and mitigating the risks associated with delayed parenthood.
