Progressive neurological disorders, such as Parkinson’s disease, amyotrophic lateral sclerosis (ALS), and multiple sclerosis (MS), present complex clinical challenges that necessitate a multidisciplinary approach for optimal patient outcomes. This review synthesizes current evidence and guideline-based practices on multidisciplinary care coordination, emphasizing its impact on care quality, symptom management, and quality of life in these populations. Recent advances, mechanism-based rationales, and practical implementation strategies are discussed, providing clinicians with actionable insights for integrating multidisciplinary care models in clinical practice.
Progressive neurological disorders are characterized by a relentless decline in neurological function, leading to substantial morbidity and healthcare utilization. The multifaceted nature of these diseases often requires seamless collaboration among various healthcare professionals, including neurologists, nurses, physiotherapists, occupational therapists, speech-language pathologists, social workers, and palliative care teams. The objective of this review is to delineate the pivotal role of multidisciplinary care coordination in managing progressive neurological disorders, examining the latest evidence, clinical guidelines, and practical considerations for implementation in real-world settings.
The incidence and prevalence of progressive neurological disorders are rising globally, predominantly owing to increasing life expectancy and improved diagnostic capabilities. Parkinson’s disease affects over 10 million people worldwide, while ALS and MS, though less common, have profound personal and societal impacts. These disorders account for a disproportionate share of disability-adjusted life years (DALYs) and healthcare expenditures. Patients frequently experience a spectrum of motor, cognitive, and behavioral symptoms, necessitating ongoing medical, rehabilitative, and psychosocial support.
Progressive neurological disorders encompass diverse pathophysiological mechanisms. For instance, Parkinson’s disease involves degeneration of dopaminergic neurons in the substantia nigra, ALS is characterized by selective motor neuron loss, and MS results from autoimmune-mediated demyelination and neurodegeneration. These mechanisms underpin the heterogeneity of clinical manifestations and progressive functional decline, reinforcing the need for individualized and multidisciplinary therapeutic strategies.
Risk factors for progressive neurological disorders are multifactorial and include genetic predispositions, environmental exposures, age, and lifestyle factors. For example, certain gene mutations (e.g., LRRK2 in Parkinson’s, SOD1 in ALS) confer increased risk, while smoking and vitamin D deficiency have been implicated in MS. Understanding these risk factors enables clinicians to identify at-risk populations and tailor preventive as well as therapeutic interventions within a multidisciplinary framework.
Clinical presentation varies widely but generally includes progressive motor impairment, such as bradykinesia, spasticity, weakness, and ataxia, as well as non-motor symptoms like cognitive decline, mood disorders, dysphagia, and autonomic dysfunction. These features often overlap and evolve over time, challenging clinicians to anticipate complications and address multifaceted patient needs through coordinated, multidisciplinary interventions.
Diagnosis of progressive neurological disorders is primarily clinical but increasingly supplemented by advanced neuroimaging, genetic testing, and biomarker analysis. Early and accurate diagnosis requires expertise from multiple disciplines, including neurology, neuroradiology, neuropsychology, and laboratory medicine. Multidisciplinary diagnostic teams facilitate comprehensive assessment, differential diagnosis, and timely intervention, reducing diagnostic delays and misclassification.
Management is inherently multidisciplinary, combining pharmacological therapies (e.g., dopaminergic agents, immunomodulators, riluzole), rehabilitative interventions (physiotherapy, occupational, and speech therapy), and psychosocial support. Regular team meetings, individualized care planning, and shared decision-making are central to addressing the dynamic needs of patients and caregivers. Integration of palliative care principles and advanced care planning is essential, especially as disease progresses and complex symptoms emerge.
Recent years have witnessed significant advances in disease-modifying therapies for MS, novel neuroprotective agents, gene therapies (e.g., for familial ALS), and technology-enabled care coordination platforms. Telemedicine and digital health tools enhance accessibility to multidisciplinary teams, particularly for patients in remote or underserved areas. Additionally, structured multidisciplinary clinics have demonstrated improvements in patient satisfaction, adherence, and health-related quality of life.
Current international and national guidelines consistently endorse multidisciplinary care coordination as a cornerstone of best practice for progressive neurological disorders. The European Federation of Neurological Societies and American Academy of Neurology recommend early multidisciplinary involvement and regular re-evaluation of care plans. Key recommendations include establishing clear communication pathways, delineating team roles, utilizing evidence-based assessment tools, and engaging patients and families in goal-setting and decision-making.
Multidisciplinary care coordination is essential in optimizing outcomes for patients with progressive neurological disorders. By integrating expertise across clinical disciplines, addressing complex symptomatology, and implementing evidence-based care models, healthcare teams can meaningfully improve the quality, efficiency, and patient-centeredness of neurological care. Ongoing research, innovation in care delivery, and guideline-driven practice will further enhance the impact of multidisciplinary approaches in this challenging and evolving field.
