Vertigo, a subjective sensation of movement, is a frequently encountered symptom in clinical practice. It poses a diagnostic challenge due to its multifactorial etiology, which necessitates a comprehensive clinical approach for appropriate management.
Vertigo is not a disease but rather a symptom of various underlying conditions. It is often the result of a mismatch of sensory inputs from the vestibular, visual, and somatosensory systems. This can be due to peripheral vestibular disorders such as benign paroxysmal positional vertigo, Meniere's disease, or central causes like stroke and multiple sclerosis.
Assessment of a patient with vertigo begins with a detailed history and physical examination. Key elements to inquire include the onset, duration, and nature of vertigo, associated symptoms, and triggering factors. Physical examination should include a thorough neurological and otologic examination, along with specific tests like the Dix-Hallpike maneuver and head impulse test.
Based on the clinical evaluation, further investigations may be needed. These could include audiometry, vestibular function tests, and neuroimaging. The choice of investigations depends on the suspected underlying etiology. For instance, magnetic resonance imaging is warranted in cases suspected of central vertigo.
Management of vertigo is tailored according to the underlying cause. It may include medications, vestibular rehabilitation exercises, or surgical interventions. Patient education and reassurance also play a critical role in managing this distressing symptom.
Vertigo is a complex symptom requiring a meticulous and systematic approach for accurate diagnosis and effective management. As healthcare professionals, understanding the pathophysiology, clinical evaluation, diagnostic approach, and management strategies of vertigo is pivotal in providing optimal patient care.
