Acute Coronary Syndrome (ACS) remains a leading cause of morbidity and mortality worldwide, necessitating the need for continuous updates in its management to optimize patient outcomes. This article provides an updated approach to ACS management, focusing on early recognition, risk stratification, and treatment strategies.
Improvements in ACS management begin with early recognition and risk stratification. High-sensitivity cardiac troponin assays have enhanced the early detection of myocardial injury, facilitating prompt intervention. Risk stratification using tools like the GRACE and TIMI risk scores help identify high-risk patients who may benefit from aggressive intervention.
Pharmacotherapy plays a pivotal role in ACS management. Dual antiplatelet therapy (DAPT), consisting of aspirin and a P2Y12 inhibitor, remains the cornerstone of treatment. Recent studies suggest that potent P2Y12 inhibitors, such as ticagrelor or prasugrel, may offer greater benefits in high-risk patients compared to clopidogrel. Furthermore, the addition of a non-statins lipid-lowering agent, like ezetimibe or PCSK9 inhibitors, to statin therapy may further reduce cardiovascular risk.
Primary percutaneous coronary intervention (PCI) is the preferred reperfusion strategy in ST-segment elevation myocardial infarction (STEMI). For non-STEMI patients, an early invasive strategy is beneficial, particularly in high-risk patients. Advances in stent technology, including the use of drug-eluting stents, have significantly reduced the risk of stent thrombosis and restenosis.
Optimizing patient outcomes in ACS involves a multifaceted approach encompassing early recognition, risk stratification, and individualized treatment strategies. It is crucial for healthcare professionals to stay abreast of the latest evidence to deliver optimal care to ACS patients. The future of ACS management is promising, with ongoing research aimed at further enhancing patient outcomes.
