This review synthesizes current evidence on implementing personalized preventive care plans across the pediatric, adolescent, adult, and geriatric life stages. Highlighting epidemiological trends, pathophysiological underpinnings, risk stratification, clinical features, diagnostic approaches, and management strategies, the article integrates recent advances and guideline updates. It provides practical insights for clinicians aiming to optimize preventive healthcare delivery in diverse populations, considering both established protocols and emerging innovations.
Preventive medicine has evolved from generalized recommendations to increasingly individualized approaches, leveraging advances in genomics, big data analytics, and behavioral science. The life course perspective recognizes that risk factors and preventive needs change dynamically with age, necessitating stage-specific strategies. Personalized preventive care aims to address unique susceptibilities and exposures at each life stage, thereby improving clinical outcomes, resource utilization, and patient engagement. This review explores the scientific underpinnings and real-world implementation of personalized preventive plans, emphasizing recent evidence and clinical applicability for healthcare professionals.
Chronic diseases remain the leading cause of morbidity and mortality globally, with risk factors often established early in life and accumulating across decades. Epidemiological studies, including the Global Burden of Disease project, reveal that over 70% of deaths are attributable to modifiable factors such as tobacco use, obesity, physical inactivity, and suboptimal nutrition. Adults require vigilant screening for hypertension, hyperglycemia, and neoplastic markers, while the elderly should be assessed for falls, cognitive impairment, and functional limitations. Early recognition of atypical presentations especially in high-risk or underrepresented populations facilitates timely and individualized preventive strategies.
Diagnosis in preventive care extends beyond disease detection to encompass risk assessment and stratification. Tools include family and personal history, validated screening questionnaires, physical examination, laboratory panels (lipids, glucose, renal and hepatic function), and increasingly, genetic and molecular testing. Age-appropriate screening protocols are essential: developmental screening in pediatrics, mental health assessments in adolescents, cancer and cardiometabolic screening in adults, and frailty and cognitive screening in geriatrics. Digital health technologies, such as wearable devices and remote monitoring, enhance longitudinal risk tracking and personalized diagnostics.
Management of risk factors is tailored by life stage and individual profile. Pediatric interventions focus on vaccination, nutrition optimization, injury prevention, and parental education. Adolescents benefit from mental health services, substance abuse counseling, and sexual health education. Adult prevention emphasizes lifestyle modification (diet, exercise, smoking cessation), pharmacologic risk reduction (statins, antihypertensives), and cancer screening. In geriatrics, management prioritizes polypharmacy review, fall prevention, social support, and advance care planning. Shared decision-making, motivational interviewing, and culturally sensitive communication are critical to maximizing adherence and effectiveness.
Precision medicine is revolutionizing preventive care through genomics, metabolomics, and the integration of artificial intelligence for risk prediction. Polygenic risk scores are being applied to forecast cardiovascular, oncologic, and metabolic disease in asymptomatic individuals. Mobile health applications enable personalized monitoring and behavioral interventions. Microbiome modulation, personalized vaccination strategies, and digital therapeutics represent other frontiers. Implementation of social determinants of health screening enables more holistic, equity-driven preventive planning. However, challenges remain regarding access, data privacy, and clinical validation of emerging tools.

Advances in genomics, digital health, and risk modeling enable more precise, effective, and patient-centered interventions. Clinicians must stay abreast of evolving evidence and guideline recommendations, ensuring that preventive plans are individualized, equitable, and grounded in robust scientific data. Ongoing research, interdisciplinary collaboration, and health system innovation will further enhance the impact of personalized prevention throughout the lifespan.
