Chronic cough, persisting for more than eight weeks, is a common yet challenging condition to manage in clinical practice due to its multifactorial nature. Despite the condition's prevalence, the diagnostic approach can be difficult, often leading to underdiagnosis or mismanagement. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough.
Understanding the etiology and pathophysiology of chronic cough is crucial for accurate diagnosis. The most common causes include gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome. However, the cough reflex is complex and involves various pathways, making the underlying pathophysiology multifaceted and often elusive.
The diagnostic approach to chronic cough should be systematic and patient-centered. A detailed clinical history and physical examination are paramount to identify potential etiologies. Objective testing such as spirometry, chest imaging, and upper gastrointestinal endoscopy may also be warranted. In cases where common causes are ruled out, consider less common etiologies like non-acid reflux, eosinophilic bronchitis, and cough hypersensitivity syndrome.
Management strategies for chronic cough should be tailored to the identified cause. Empiric treatment trials for the most common causes can be an effective strategy. For refractory cases, multidisciplinary management involving pulmonologists, gastroenterologists, and otolaryngologists may be beneficial. Emerging therapies targeting neuronal pathways involved in cough reflex are also promising.
Chronic cough is a complex condition requiring a comprehensive and systematic approach for effective diagnosis and management. Understanding the multifactorial nature of chronic cough, adopting a patient-centered diagnostic approach, and tailoring management strategies to the individual patient are vital. Future research should focus on unraveling the intricate pathways involved in the cough reflex and developing targeted therapies.
