Laboratory tests should include a complete blood count (CBC), reticulocyte count, and peripheral smear.
Anemia can be classified based on the mean corpuscular volume (MCV) into microcytic, normocytic, and macrocytic anemia. This classification helps narrow down the potential causes and guide further investigations.
Further investigations depend on the initial classification. Microcytic anemia may suggest iron deficiency or thalassemia, warranting iron studies or hemoglobin electrophoresis. Normocytic anemia may be due to chronic diseases, hemolysis, or marrow failure, necessitating tests such as reticulocyte count, haptoglobin, lactate dehydrogenase (LDH), and bone marrow examination. Macrocytic anemia may be caused by vitamin B12 or folate deficiency, myelodysplastic syndrome, or alcoholism, requiring tests like serum B12, folate levels, and bone marrow examination.
If the cause of anemia remains elusive after these investigations, referral to a hematologist may be warranted. This is particularly important in cases suggestive of bone marrow disorders or malignancies.
