Electrocardiogram (ECG) interpretation is a fundamental skill in clinical medicine. Despite advances in imaging and molecular diagnostics, the ECG remains an invaluable tool in the evaluation of cardiac pathology. This article aims to provide a comprehensive guide to ECG interpretation in everyday medical practice.
An ECG is a graphical representation of the electrical activity of the heart. It is essential to understand the basic cardiac anatomy, physiology, and the conduction system to interpret ECGs accurately. Each wave, segment, and interval on an ECG has a specific meaning and correlates with a particular phase of cardiac electrical activity.
ECG interpretation requires a systematic approach. Firstly, identify the rhythm by examining the P waves and QRS complexes. Secondly, calculate the heart rate. Next, assess the axis by analyzing the QRS complex in the limb leads. Fourthly, look for any conduction delays or blocks. Finally, evaluate for signs of ischemia, infarction, or other pathologies by scrutinizing the ST segments, T waves, and Q waves.
Common ECG abnormalities include arrhythmias, conduction blocks, and signs of ischemia or infarction. Arrhythmias may manifest as irregular rhythms, abnormal P waves, or bizarre QRS complexes. Conduction blocks often result in prolonged intervals or missing waves. Ischemia typically causes ST segment depression or T wave inversion, while infarction leads to ST segment elevation or pathological Q waves.
ECG interpretation is a crucial skill that all physicians should master. A systematic approach, combined with a thorough understanding of the underlying cardiac physiology, can help clinicians identify a wide range of cardiac pathologies. Despite the advent of advanced diagnostic tools, the ECG remains a rapid, cost-effective, and invaluable tool in everyday medical practice.
