The increasing use of neck ultrasonography has led to a rise in the detection of thyroid nodules, necessitating a systematic approach to their evaluation. This guide aims to provide a comprehensive overview of thyroid nodule assessment, highlighting its intricacies and recent advances.
Thyroid nodules are often discovered incidentally during physical examination or imaging studies. A thorough history and physical examination, focusing on risk factors for malignancy, is the initial step in evaluation. High-resolution neck ultrasound is the imaging modality of choice, providing information about nodule size, characteristics, and the presence of suspicious cervical lymph nodes.
Following initial evaluation, the decision to further investigate a nodule is guided by its ultrasound characteristics and patient's risk factors. Fine-needle aspiration biopsy (FNAB) is the most reliable method for distinguishing benign from malignant nodules. Molecular testing can be useful when FNAB results are indeterminate.
Most thyroid nodules are benign and require only periodic monitoring. However, nodules with suspicious features or those causing compressive symptoms may require surgical intervention. Decision for surgery should be individualized, considering patient's overall health, risk of malignancy, and preference.
Advancements in molecular testing have improved the diagnostic accuracy of FNAB. Additionally, the emergence of active surveillance as a management option for low-risk papillary thyroid carcinomas has changed the treatment paradigm, reducing unnecessary surgeries and their associated morbidity.
Thyroid nodule evaluation is a nuanced process, requiring a systematic approach and judicious use of diagnostic modalities. With the advent of new technologies and management strategies, healthcare professionals can optimize patient outcomes while minimizing risks.
