Acute chest pain is a common symptom that often prompts patients to seek immediate medical attention. It presents a diagnostic challenge due to its diverse etiology, ranging from benign conditions to life-threatening diseases such as acute myocardial infarction or pulmonary embolism.
Initial assessment should include a thorough history and physical examination to identify any high-risk features. The physician should also consider the patient's age, gender, and risk factors for coronary artery disease. Diagnostic tests such as electrocardiogram (ECG) and biomarkers like troponin can provide valuable information about the potential cardiac origin of the chest pain.
The differential diagnosis of acute chest pain is broad and includes cardiac, gastrointestinal, pulmonary, and musculoskeletal conditions. Life-threatening causes must be ruled out first, such as acute coronary syndrome, aortic dissection, and pulmonary embolism. Other non-cardiac causes such as gastroesophageal reflux disease or costochondritis should also be considered.
Management of acute chest pain is guided by the likely diagnosis. In suspected acute coronary syndrome, immediate reperfusion therapy is crucial. For non-cardiac causes, appropriate treatment should be initiated based on the underlying condition. All patients should be educated about the warning signs of serious conditions and advised to seek immediate medical attention if symptoms recur.
Acute chest pain is a complex clinical symptom that requires a systematic and comprehensive approach. Early recognition and appropriate management of life-threatening conditions can significantly improve patient outcomes. Continued education and training in the evaluation and management of acute chest pain is essential for all healthcare professionals.
