A thorough history taking and physical examination is the cornerstone of the initial evaluation. It is crucial to identify any changes in appetite, dietary habits, psychiatric symptoms, or medications that may contribute to weight loss. The physical examination should focus on signs of malnutrition, wasting syndromes, and systemic diseases.
Basic laboratory tests, including complete blood count, comprehensive metabolic panel, thyroid function tests, and inflammatory markers, are recommended in the initial workup. If these tests are inconclusive, advanced diagnostic tools such as imaging studies, endoscopies, or biopsies may be necessary.
Once the underlying cause is identified, management should be condition-specific and patient-centered. In cases where no cause is found, symptomatic management with nutritional support and appetite stimulants can be beneficial. Regular follow-up is essential to monitor the patient's progress and adjust treatment plans accordingly.
Unexplained weight loss is a complex clinical problem that requires a comprehensive approach for effective evaluation and management. A systematic and methodical approach can help identify the underlying cause, enabling appropriate, timely interventions. It is essential to remember that despite exhaustive investigations, a cause may not always be found, and symptomatic management becomes crucial.
