Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight over a period of 6-12 months without a clear cause, is a common yet perplexing issue encountered in clinical practice. It can be the harbinger of a serious underlying illness, making its evaluation crucial for clinicians.
UWL can be attributed to myriad causes, broadly categorized into three groups: malignancy, non-malignant gastrointestinal diseases, and psychiatric disorders. Other causes include endocrine disorders, chronic infections, and drugs. However, in approximately 15-25% of cases, no cause is identified despite extensive evaluation.
The initial assessment should involve a comprehensive history and physical examination, focusing on symptoms of potential underlying conditions. Detailed dietary history, drug history, and psychosocial assessment are also essential. If the initial evaluation does not reveal a cause, laboratory tests, imaging studies, and endoscopic procedures may be warranted based on clinical suspicion.
Diagnostic imaging plays a pivotal role in the evaluation of UWL. Computed tomography (CT) of the chest, abdomen, and pelvis is often the first-line imaging modality, given its ability to detect malignancies and other systemic diseases. However, the choice of imaging should be guided by individual patient characteristics and clinical suspicion.
The management of UWL is primarily directed towards treating the underlying cause. In cases where no cause is identified, symptomatic management and regular follow-up are recommended. The follow-up strategy should be individualized based on the patient's age, comorbidities, and clinical progress.
Unexplained weight loss is a complex clinical issue that requires a systematic and comprehensive approach for evaluation. A thorough history, physical examination, and appropriate use of diagnostic tools are key to identifying the underlying cause. In cases where no cause is identified, a patient-centered approach focusing on symptom management and regular follow-up is essential.
