Abstract

Frailty screening in the emergency department (ED) is gaining prominence as a pivotal strategy for rapid care prioritization among older adults. Accurately identifying frailty at the point of ED entry enables clinicians to tailor management, anticipate complications, and improve patient outcomes. This review evaluates the epidemiology, pathophysiology, risk factors, clinical features, diagnostic approaches, and management strategies for frailty screening in the ED, integrating current evidence, emerging advances, and recent guideline recommendations to support best practices in acute care settings.

Introduction

With a globally aging population, emergency departments are increasingly confronted with complex presentations of older adults, many of whom are living with frailty. Frailty is a multidimensional syndrome characterized by diminished physiological reserve and increased vulnerability to stressors, resulting in adverse health outcomes. The rapid and accurate identification of frailty in the ED is essential for optimal risk stratification, resource allocation, and personalized care. Integrating structured frailty assessments into ED workflows is supported by a growing body of evidence and aligns with contemporary models of geriatric emergency medicine.

Epidemiology / Disease Burden

Diagnosis

Several validated assessment tools facilitate frailty screening in the ED. The Clinical Frailty Scale (CFS), Frailty Index, and the Identification of Seniors at Risk (ISAR) tool are among the most widely used, offering rapid bedside applicability. The CFS, for example, stratifies patients based on functional status and comorbidity, with higher scores correlating with worse outcomes. Recent evidence supports the use of brief screening tools integrated into triage protocols to identify high-risk individuals, enabling early multidisciplinary input and resource prioritization. Biomarker research (e.g., inflammatory markers, sarcopenia indices) holds promise but remains largely investigational in acute settings.

Treatment & Management

Management of frailty in the ED involves immediate stabilization, identification and treatment of reversible causes, and tailored disposition planning. Comprehensive Geriatric Assessment (CGA) is the gold standard, but its application may be limited in time-pressured environments. Key interventions include early mobilization, medication review (deprescribing unnecessary drugs), nutritional support, and cognitive screening. Multidisciplinary collaboration is essential, with input from geriatricians, pharmacists, physiotherapists, and social services. Transition-of-care planning, including communication with primary care and community resources, reduces readmission and improves functional outcomes.

Recent Advances / Emerging Therapies

Emerging strategies in ED frailty screening include the implementation of digital decision-support tools, electronic health record (EHR) integration of frailty indices, and remote assessment technologies. Artificial intelligence-driven risk stratification models are being piloted to enhance predictive accuracy. Mobile apps and digital frailty checklists offer rapid, standardized assessment, supporting clinical workflow without increasing administrative burden. Additionally, recent trials are evaluating targeted interventions such as acute exercise protocols and immunonutrition in frail ED patients, with promising early results.

Guideline Recommendations

International guidelines, including those from the American College of Emergency Physicians (ACEP) and the British Geriatrics Society, emphasize the routine incorporation of frailty screening into ED triage for older adults. Recommendations highlight the use of validated tools (e.g., CFS, ISAR), the importance of early geriatrician involvement, and the implementation of tailored care pathways for frail individuals. There is consensus on the need for multidisciplinary approaches and the integration of frailty data into discharge planning and community follow-up.
