Thyroid nodules, a common clinical finding, pose a significant challenge due to their potential malignancy. This article aims to provide a comprehensive guide to their evaluation, aiding healthcare professionals in their diagnostic approach.
Initial evaluation should include a thorough history and physical examination, focusing on risk factors for malignancy. These include exposure to radiation, family history of thyroid cancer, and presence of symptoms such as dysphagia, hoarseness, and rapid nodule growth.
Ultrasound remains the primary imaging modality for thyroid nodules due to its high sensitivity in detecting nodular characteristics indicative of malignancy. Suspicious features include microcalcifications, irregular margins, taller-than-wide shape, and hypoechogenicity.
Fine-needle aspiration (FNA) biopsy is recommended for nodules >1cm with suspicious ultrasound features, or >1.5cm in the absence of such features. FNA allows for cytologic examination, which can guide the need for surgery.
When cytology results are indeterminate, molecular testing may be beneficial. Tests such as Afirma, Thyroseq, and RosettaGX reveal can provide further insight into the risk of malignancy, aiding in decision-making regarding surgical intervention.
Management depends on the risk of malignancy. Low-risk nodules may be monitored with serial ultrasounds, while high-risk nodules typically warrant surgical removal. Radioactive iodine or ethanol ablation may be considered in select cases.
Thyroid nodule evaluation is a complex process requiring careful risk assessment, appropriate use of imaging and biopsy, and consideration of genetic testing. By understanding these complexities, healthcare professionals can better navigate the diagnostic pathway, leading to improved patient outcomes.
