As fibrotic tissue replaces healthy liver cells, blood flow is impeded, causing portal hypertension. This can lead to varices, ascites, and hepatic encephalopathy. Management includes beta-blockers to lower portal pressure and endoscopic procedures for variceal bleeding.
The liver's role in producing clotting factors means cirrhosis can lead to coagulopathy. This presents a delicate balance between bleeding and clotting risks. Management involves careful monitoring and judicious use of fresh frozen plasma, vitamin K, and platelet transfusion.
Ammonia accumulation due to impaired hepatic function can lead to hepatic encephalopathy. Lactulose and Rifaximin are mainstays of treatment, reducing gut ammonia production and absorption respectively.
Malnutrition is common in cirrhotic patients due to decreased nutrient absorption and altered metabolism. Nutritional support and supplementation, including branched-chain amino acids, are crucial components of management.
Cirrhosis significantly increases the risk of hepatocellular carcinoma (HCC). Regular surveillance via ultrasound and alpha-fetoprotein levels is essential. If detected early, curative treatments like resection, transplantation, or ablation may be possible.
Managing liver cirrhosis and its complications requires a comprehensive, multidisciplinary approach. Healthcare professionals must be well-versed in the various complications and their management to provide optimal care. Further research into novel therapeutic strategies will continue to improve outcomes for patients with this complex disease.
