Enteric fever, caused by Salmonella enterica serovars Typhi and Paratyphi, remains a significant global health concern. This article aims to provide a comprehensive guide to an integrated approach for its diagnosis and clinical management.
Diagnosing enteric fever involves isolating the bacteria from blood, bone marrow, or specific lesions. However, these methods can be time-consuming and lack sensitivity. Recent advances include molecular diagnostic techniques, such as polymerase chain reaction (PCR), which offer rapid and accurate identification. Serological tests, while less specific, can be useful in low-resource settings or when cultures are unavailable.
Antimicrobial therapy remains the cornerstone of enteric fever management. Fluoroquinolones, third-generation cephalosporins, and azithromycin are commonly used. However, increasing antimicrobial resistance necessitates regular susceptibility testing and judicious use of antibiotics.
Supportive care plays a critical role in managing complications and improving patient outcomes. This includes hydration, nutritional support, and management of symptoms such as fever and pain. Close monitoring for potential complications, such as intestinal perforation or neurologic manifestations, is essential.
Vaccination is a crucial preventive measure, particularly in endemic regions. Two vaccines are currently available: the injectable Vi polysaccharide vaccine and the oral Ty21a vaccine. Both have demonstrated efficacy, although neither provides complete protection.
The management of enteric fever requires a comprehensive, integrated approach that considers accurate diagnosis, effective antimicrobial therapy, supportive care, and preventive measures such as vaccination. As the landscape of antimicrobial resistance evolves, ongoing research and development of new diagnostic methods, therapeutics, and vaccines will be crucial to maintaining progress against this global health threat.
