Unexplained weight loss (UWL) is a common yet perplexing clinical scenario that often presents a diagnostic challenge for healthcare professionals. It is defined as the unintentional loss of more than 5% of body weight within a six-month period. This article aims to provide a comprehensive approach to the evaluation of UWL in clinical practice.
UWL can be indicative of numerous underlying conditions, including malignancies, gastrointestinal disorders, endocrine diseases, and psychiatric disorders. In some cases, it may be the only symptom of a serious underlying condition. Therefore, a thorough and systematic approach to the evaluation of UWL is essential.
The first step in the evaluation of UWL is a comprehensive history and physical examination. The history should include questions about the duration and magnitude of weight loss, associated symptoms, dietary intake, and psychosocial factors. The physical examination should focus on signs of malnutrition and potential clues to underlying diseases.
Following the initial assessment, targeted laboratory and imaging studies should be performed based on the findings from the history and physical examination. These may include complete blood count, comprehensive metabolic panel, thyroid function tests, and imaging studies such as computed tomography (CT) scan or endoscopy.
Treatment of UWL should be directed towards the underlying cause, if identified. In cases where no specific cause is found, symptomatic and supportive care should be provided, including nutritional support and symptom management.
UWL is a complex clinical issue that requires a systematic and comprehensive approach for its evaluation. By understanding the potential etiologies and maintaining a methodical approach to clinical assessment, laboratory and imaging studies, healthcare professionals can effectively diagnose and manage patients presenting with UWL.
