Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight over a period of 6 to 12 months without a clear cause, can be a perplexing clinical scenario. It may be an initial manifestation of a serious underlying disease, thus necessitating a comprehensive evaluation.
The first step is a detailed history and physical examination. It is crucial to ascertain the timeline, associated symptoms, dietary habits, and psychosocial factors. The physical examination should focus on signs of malnutrition, malignancy, and systemic diseases.
Initial laboratory investigations may include complete blood count, metabolic panel, thyroid function tests, and inflammatory markers. Based on these results and the clinical presentation, further targeted investigations may be required.
Imaging studies are not routinely recommended unless directed by specific symptoms, signs, or laboratory abnormalities. Computed tomography (CT) or magnetic resonance imaging (MRI) could be considered in cases with high suspicion of malignancy or systemic disease.
Management is primarily directed towards the underlying cause, if identified. In cases where no clear cause is found, symptomatic treatment and nutritional support are vital. Regular follow-up is necessary to monitor the patient's progress and reassess the need for further investigations.
UWL is a complex clinical scenario that requires a systematic and comprehensive approach. While it can be a daunting task, a meticulous history, thorough physical examination, and judicious use of investigations can often lead to the identification of the underlying cause. In cases where the cause remains elusive, supportive care and close follow-up are essential to ensure optimal patient outcomes.
