A systematic approach to ECG interpretation can prevent missed diagnoses. Begin by checking the calibration and recording speed. Next, assess the rhythm, followed by the rate. Then, examine the P wave, PR interval, QRS complex, ST segment, T wave, and QT interval in that order. Finally, consider the overall clinical context.
Common ECG abnormalities include arrhythmias, myocardial infarction, and electrolyte imbalances. Arrhythmias may manifest as irregular rhythm or abnormal rate. Myocardial infarction often presents with ST segment elevation or depression. Electrolyte imbalances can cause changes in the T wave or QT interval.
ECG interpretation can be challenging due to variations in patient physiology and technical issues. For instance, left ventricular hypertrophy or bundle branch blocks can alter the ECG waveform. Artifacts due to patient movement or electrical interference can also complicate interpretation.
Mastering ECG interpretation requires a solid understanding of the basics, a systematic approach, and the ability to recognize common abnormalities.
