This article synthesizes current evidence, mechanisms, and guideline-based recommendations, offering a structured approach to ambiguous behavioral and psychological symptoms. Drawing on recent advances, clinical insights, and practical implications, we aim to enhance the diagnostic acumen and management strategies of clinicians encountering complex cases.
The clinical landscape of behavioral and psychological symptoms is characterized by heterogeneity and diagnostic uncertainty. Patients frequently present with overlapping or non-specific symptoms such as agitation, mood disturbances, or cognitive changes, complicating the identification of underlying etiologies. The increasing prevalence of neuropsychiatric comorbidities, coupled with an aging population and diverse sociocultural backgrounds, underscores the importance of robust diagnostic frameworks. Case-based learning (CBL) provides a structured, interactive method for clinicians to navigate the intricacies of ambiguous presentations, promoting critical thinking, pattern recognition, and evidence-informed decision-making.
Ambiguous behavioral and psychological presentations are encountered across all age groups and clinical settings, with particular prominence in primary care, psychiatry, neurology, and geriatrics. Epidemiological studies highlight that up to 60% of elderly inpatients exhibit behavioral and psychological symptoms of dementia (BPSD), while non-specific psychiatric presentations account for nearly 30% of psychiatric consultations in general hospitals. The societal and economic burden is substantial, leading to increased hospitalizations, prolonged length of stay, caregiver distress, and higher healthcare costs. Diagnostic evaluation of ambiguous behavioral and psychological symptoms requires a systematic, multi-modal approach. Initial assessment should include detailed history, mental status examination, neurological evaluation, and targeted physical examination. Laboratory investigations (CBC, electrolytes, liver/renal function, thyroid profile, vitamin B12, drug levels) are essential to exclude reversible causes.
