Fatigue is a subjective feeling of tiredness or lack of energy, often associated with decreased physical and cognitive performance. It can be acute or chronic, with chronic fatigue lasting more than six months. The etiology of fatigue is multifaceted, encompassing physical, psychological, and social factors.
A comprehensive patient history and physical examination are crucial in the assessment of fatigue. The history should focus on the onset, duration, severity, and impact of fatigue on daily activities. Any associated symptoms, medical history, medication use, lifestyle factors, and psychosocial stressors should also be evaluated. Physical examination should aim to identify any signs of systemic disease.
Investigations should be guided by the clinical history and physical examination findings. Basic investigations may include complete blood count, thyroid function tests, liver function tests, and urine analysis. Further investigations may be warranted based on the initial findings.
Management of fatigue should be individualized, addressing the underlying cause(s). This may involve medical management for identified disease, lifestyle modification, psychological intervention, or a combination of these approaches. Regular follow-up is essential to monitor progress and adjust management as needed.
Fatigue can significantly impact a patient's quality of life. A comprehensive clinical approach, integrating a thorough history, physical examination, and appropriate investigations, is key to identifying the underlying cause and providing effective management. Continued education and research are necessary to further our understanding and management of this complex symptom.
