The management of peptic ulcer disease (PUD) has evolved significantly over the past few decades, driven by advancements in diagnostic techniques, treatment modalities, and our understanding of the pathogenesis of the disease. This article provides a comprehensive overview of these advancements and their implications for healthcare professionals.
Research has identified Helicobacter pylori infection and non-steroidal anti-inflammatory drugs (NSAIDs) as the two primary causes of PUD. Understanding these etiological factors has revolutionized PUD management, with eradication of H. pylori and minimization of NSAID use now forming the cornerstone of treatment.
Endoscopy remains the gold standard for diagnosing PUD. However, non-invasive tests such as urea breath test and stool antigen test for H. pylori have simplified the diagnostic process. Serology, although less accurate, can be useful in resource-limited settings.
Triple therapy, comprising a proton pump inhibitor (PPI) and two antibiotics, has been the standard treatment for H. pylori-associated PUD. However, due to increasing antibiotic resistance, quadruple therapy including a PPI, bismuth, and two antibiotics is now recommended. For NSAID-induced ulcers, discontinuation of NSAIDs, if possible, and PPI therapy are advised.
Prophylaxis with PPIs or H2 receptor antagonists is recommended for high-risk patients on NSAIDs. Additionally, the use of selective cyclooxygenase-2 inhibitors can reduce the risk of PUD in these patients. Vaccination against H. pylori is a promising preventive measure, but it is still in the research phase.
Advancements in the understanding, diagnosis, and treatment of PUD have significantly improved patient outcomes. However, the increasing prevalence of antibiotic resistance poses a challenge, necessitating further research in this field. Healthcare professionals must stay abreast of these developments to provide optimal care for patients with PUD.
