The intricate interplay between the endocrine and immune systems is increasingly recognized as a fundamental determinant of metabolic tissue regulation. Recent discoveries underscore the bidirectional communication channels that exist between hormonal signals and immune mediators within adipose tissue, skeletal muscle, and the liver, influencing metabolic homeostasis, inflammation, and susceptibility to metabolic diseases. This review synthesizes current research, clinical relevance, and emerging therapeutic strategies targeting endocrine-immune crosstalk, offering a comprehensive overview for clinicians and researchers.
Metabolic tissues such as adipose tissue, skeletal muscle, and liver are not only pivotal for energy storage and nutrient metabolism but also serve as active immunoendocrine organs. The crosstalk between the endocrine and immune systems orchestrates responses to metabolic stress, infection, and injury. Disruption of these interactions underlies many metabolic disorders, including obesity, type 2 diabetes mellitus (T2DM), and non-alcoholic fatty liver disease (NAFLD). Understanding the molecular and cellular mechanisms governing endocrine-immune communication is essential for developing precision therapies addressing the metabolic disease epidemic.
The burden of metabolic disorders characterized by dysregulated endocrine-immune crosstalk is rapidly escalating worldwide. Obesity affects over 650 million adults globally, with T2DM impacting more than 460 million. NAFLD prevalence exceeds 25% in many populations. These conditions are associated with chronic low-grade inflammation, altered cytokine profiles, and immune cell infiltration in metabolic tissues, highlighting the clinical significance of immune-endocrine dysregulation.
The pathophysiological basis of metabolic tissue dysfunction involves a dynamic dialogue between hormonal signals (such as insulin, leptin, adiponectin, and glucocorticoids) and immune mediators (including cytokines, chemokines, and immune cell-derived factors). In obesity, hypertrophic adipocytes secrete increased pro-inflammatory adipokines, while infiltrating macrophages and T cells exacerbate local and systemic inflammation. Insulin resistance is partly driven by immune-mediated serine phosphorylation of insulin receptor substrate proteins. Conversely, anti-inflammatory cytokines and regulatory immune cells can restore metabolic sensitivity, highlighting therapeutic opportunities in modulating this crosstalk.
Genetic predisposition, sedentary lifestyle, high-calorie diets, and chronic stress are major risk factors for dysregulated endocrine-immune interactions. Age, sex hormones, and gut microbiota composition further modulate immune responses in metabolic tissues. Additionally, environmental pollutants and certain medications may disrupt immunometabolic signaling, increasing susceptibility to metabolic diseases.
Patients with endocrine-immune dysregulation often present with features of metabolic syndrome, including central obesity, insulin resistance, dyslipidemia, and hypertension. Systemic inflammation manifests as elevated C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α) levels. In advanced cases, hepatic steatosis, muscle wasting, and impaired wound healing reflect widespread immunometabolic disturbances.
Diagnosis relies on a combination of clinical assessment, biochemical markers, and imaging. Evaluation includes fasting glucose, insulin, lipid profile, HbA1c, CRP, and adipokine levels. Imaging modalities such as MRI and ultrasound assess tissue-specific inflammation and fat deposition. Flow cytometry and transcriptomic profiling of tissue biopsies enable in-depth characterization of immune cell populations and molecular signatures in metabolic organs.
Therapeutic strategies focus on lifestyle modification, pharmacologic interventions, and targeted immunomodulation. Weight loss through diet and physical activity remains foundational. Pharmacotherapies include insulin sensitizers (metformin, thiazolidinediones), GLP-1 receptor agonists, and SGLT2 inhibitors, which may exert anti-inflammatory effects. Emerging approaches target immune pathways, such as IL-1β antagonists and regulatory T cell enhancers, to rebalance immunometabolic signaling.
Recent advances highlight the role of innate lymphoid cells, myeloid-derived suppressor cells, and novel adipokines in modulating tissue inflammation and metabolic function. Clinical trials of immune-modulating biologics (e.g., canakinumab, anakinra) show promise for treating metabolic complications. Gene editing and RNA-based therapies targeting key immunometabolic regulators are under investigation. Precision medicine approaches integrating multi-omic data are poised to refine patient stratification and therapy selection.
Recent guidelines from leading endocrinology and diabetes associations emphasize the importance of early detection and management of metabolic inflammation. Multidisciplinary care, individualized risk assessment, and aggressive management of cardiovascular risk factors are recommended. Monitoring inflammatory biomarkers and considering immunomodulatory therapies in select patients are emerging considerations in guideline updates.
The endocrine-immune crosstalk in metabolic tissues represents a critical determinant of health and disease. Ongoing research continues to unravel the cellular and molecular underpinnings of these interactions, offering new avenues for intervention in metabolic disorders. Clinicians should maintain vigilance for signs of immunometabolic dysregulation and adopt integrated management strategies to improve patient outcomes in this rapidly evolving field.
