It is crucial to consider the potential etiologies, which can broadly be classified into infectious, allergic, autoimmune, or drug-induced. Infectious causes include viral exanthems, bacterial infections, or vector-borne diseases. Allergic reactions, autoimmune diseases like Systemic Lupus Erythematosus, and drug reactions are non-infectious causes.
A systematic approach begins with a detailed history and physical examination. Key elements of history include travel, exposure, medication use, and symptom chronology. The rash's characteristics such as morphology, distribution, and progression, along with systemic signs, guide the differential diagnosis.
Investigations should be guided by the clinical suspicion. Complete blood count, liver and renal function tests, and inflammatory markers are usually the first-line investigations. Specific serological tests, skin biopsy, or cultures may be required based on the suspected etiology.
Management is etiology-specific and may range from symptomatic treatment in self-limiting conditions to specific antimicrobial therapy or immunosuppression. In cases of drug-induced fever with rash, drug discontinuation is usually sufficient.
