The concept of "Agni" digestive and metabolic fire in Ayurveda offers a framework for tailoring dietary recommendations to individual constitutional and physiological profiles. This review synthesizes current clinical guidelines, mechanistic insights, and evidence-based practices for Agni-based nutritional personalization, emphasizing its potential integration into contemporary medical practice. The article systematically discusses epidemiological trends, pathophysiological mechanisms, risk factors, clinical features, diagnostic approaches, management strategies, recent advances, and expert-driven recommendations, with a focus on clinical relevance for healthcare professionals.
The paradigm shift from generalized dietary advice to individualized nutrition underscores the need for nuanced clinical guidelines. In Ayurveda, Agni represents the central mechanism governing digestion, metabolism, and assimilation of nutrients. Imbalances in Agni are believed to precipitate a spectrum of metabolic and systemic disorders. Recent research highlights the potential of integrating Agni-based assessments with contemporary nutritional science, offering promising avenues for personalized interventions. This article delineates the current state of evidence and provides actionable recommendations for clinicians seeking to incorporate Agni-based nutritional personalization into practice.
Non-communicable diseases (NCDs) such as obesity, diabetes, metabolic syndrome, and gastrointestinal disorders constitute a significant global health burden, with dietary factors playing a decisive role in their pathogenesis. Epidemiological studies reveal substantial inter-individual variability in dietary response, metabolic adaptation, and disease manifestation. Traditional approaches often overlook this heterogeneity, resulting in suboptimal outcomes. Agni-based frameworks, when mapped to contemporary nutrition science, may offer a means to stratify patients and enhance preventive and therapeutic efficacy, particularly in regions with high prevalence of metabolic and digestive disorders.
According to Ayurvedic principles, Agni is classified into four main types Samagni (balanced), Vishamagni (irregular), Tikshnagni (hyperactive), and Mandagni (hypoactive) each associated with distinctive metabolic and physiological features. Disruption of Agni leads to impaired digestion, toxin accumulation (Ama), and disturbed homeostasis. Modern research correlates Agni imbalances with alterations in gut microbiota, enzymatic activity, and metabolic flexibility. Tikshnagni management emphasizes cooling and nourishing foods, whereas Mandagni requires light, easily digestible, and metabolism-enhancing dietary modifications.
