Abstract

Chronic skin disorders, such as psoriasis, atopic dermatitis, and vitiligo, require ongoing monitoring and management due to their relapsing-remitting nature and impact on patient quality of life. Traditional face-to-face follow-up can be resource-intensive and inconvenient for both patients and healthcare systems. Digital long-term follow-up models have emerged as innovative solutions, leveraging telemedicine, mobile health applications, and remote monitoring technologies. This article reviews the evidence, mechanisms, clinical implications, and guideline recommendations for implementing digital follow-up in chronic skin disorders, highlighting both the benefits and challenges of this evolving paradigm.

Introduction

Chronic dermatologic diseases impose significant morbidity and often necessitate recurring, lifelong follow-up. In the era of digital health transformation, remote monitoring and teledermatology have gained traction as alternatives or adjuncts to conventional care. Driven by technological advances, patient preferences, and recent global health crises, digital models promise to optimize clinical outcomes, improve access, and reduce healthcare costs. This review critically appraises the scientific rationale, clinical utility, and future directions for digital long-term follow-up in chronic skin disorders, with a focus on evidence-based practice.

Epidemiology / Disease Burden

Chronic skin conditions are prevalent worldwide. Psoriasis affects 2–3% of the global population, while atopic dermatitis impacts up to 20% of children and 3% of adults. Other disorders, such as chronic urticaria and hidradenitis suppurativa, also contribute substantially to dermatologic disease burden. Chronic skin diseases can wax and wane, with periods of remission and exacerbation. Monitoring disease activity, treatment adherence, and quality of life is essential. Digital tools enable continuous data capture, photographic documentation, and patient engagement, facilitating individualized and timely care adjustments.

Diagnosis

Diagnosis is primarily clinical, supported by history, physical examination, and occasionally histopathology or laboratory tests. Digital approaches, such as teledermatology, allow for remote assessment using high-resolution images and validated scoring systems (e.g., PASI for psoriasis, SCORAD for atopic dermatitis). Automated algorithms and artificial intelligence are being investigated to augment diagnostic accuracy and triage, though human expertise remains indispensable.

Treatment & Management

Management strategies are tailored to disease severity and patient needs, encompassing topical therapies, phototherapy, systemic agents, and biologics. Long-term follow-up is crucial to monitor efficacy, adverse events, and comorbid conditions. Digital models provide platforms for treatment reminders, side effect reporting, medication reconciliation, and adherence monitoring, potentially improving outcomes and patient satisfaction. Remote consultations can facilitate timely escalation or de-escalation of therapy based on objective and subjective data.

Recent Advances / Emerging Therapies

Recent years have witnessed the proliferation of digital health technologies in dermatology. Mobile apps for symptom tracking, automated lesion measurement, and treatment adherence have shown promise in pilot studies. Wearable sensors and smart devices can monitor physiological parameters, while AI-driven decision support tools are being integrated into teledermatology workflows. Early evidence suggests that digital follow-up can reduce unnecessary clinic visits, enhance disease control, and empower patients through education and self-management. However, issues such as data security, digital literacy, and interoperability remain to be addressed.

Guideline Recommendations

International and national guidelines increasingly acknowledge the role of telemedicine and digital follow-up in chronic skin disorders. The American Academy of Dermatology and European Dermatology Forum recommend incorporating telehealth for routine check-ins, particularly for stable patients or those on maintenance therapy. Guidelines emphasize the need for robust clinical protocols, patient selection criteria, and quality assurance measures to ensure safety and efficacy. They also advocate for hybrid models combining digital and in-person care to optimize outcomes and address complex cases.

Digital long-term follow-up models represent a transformative advance in the management of chronic skin disorders, offering scalable, patient-centered, and cost-effective solutions. By leveraging telemedicine, mobile apps, and remote monitoring, clinicians can deliver continuous, adaptive care that aligns with current evidence and evolving patient needs. While challenges remain such as ensuring equitable access, data privacy, and clinical validation continued research and guideline development will be key to unlocking the full potential of digital dermatology in the chronic care paradigm.
