Ascites and Spontaneous Bacterial Peritonitis (SBP)

Ascites, the most common cirrhosis complication, often leads to spontaneous bacterial peritonitis. Management includes sodium restriction, diuretics, and therapeutic paracentesis. Antibiotic prophylaxis is crucial for preventing SBP in high-risk patients.

Hepatic Encephalopathy (HE)

HE manifests as a spectrum of neuropsychiatric abnormalities. Treatment aims at reducing serum ammonia levels, typically with lactulose and, in refractory cases, rifaximin. Regular monitoring of mental status is essential.

Portal Hypertension and Varices

Portal hypertension can lead to the formation of varices, which may rupture causing life-threatening bleeding. Beta-blockers and endoscopic band ligation are the mainstays of primary and secondary prophylaxis.

Hepatorenal Syndrome (HRS)

HRS, a serious complication, leads to renal failure in the absence of other kidney diseases. Management includes volume expansion, vasoconstrictors, and ultimately, liver transplantation.

Hepatocellular Carcinoma (HCC)

Patients with cirrhosis are at high risk for HCC. Regular surveillance with ultrasound and alpha-fetoprotein levels is crucial. Treatment options include surgical resection, liver transplantation, and locoregional therapies.
