Chronic cough, a persistent cough lasting longer than eight weeks, is a common yet challenging condition to manage in clinical practice. Despite its prevalence, the multifaceted nature of this condition often leads to diagnostic uncertainty and therapeutic complexity.
When approaching a chronic cough, a comprehensive clinical history and physical examination are paramount. Clinicians should consider common etiologies such as gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and asthma. However, it's crucial to bear in mind less common causes like lung cancer, sarcoidosis, or foreign body aspiration. A meticulous review of the patient's medication regimen is also essential, as drugs like angiotensin-converting enzyme inhibitors can induce coughing.
Diagnostic testing should be guided by clinical suspicion. Chest radiographs and spirometry can provide valuable insights into pulmonary pathology. In cases where GERD or UACS is suspected, pH monitoring or sinus imaging might be warranted. However, indiscriminate ordering of tests can lead to unnecessary expenses and patient anxiety.
Treatment should be tailored to the identified etiology. For example, proton pump inhibitors may be beneficial for GERD-associated cough, while inhaled corticosteroids can be effective for asthma-related cough. In refractory cases, multidisciplinary collaboration with pulmonologists, otolaryngologists, or gastroenterologists may be necessary to optimize patient outcomes.
Chronic cough is a complex condition requiring a comprehensive and systematic approach for effective management. By maintaining a high index of suspicion for common and uncommon causes, utilizing judicious diagnostic testing, and implementing targeted treatment strategies, clinicians can effectively manage this challenging condition and improve patient quality of life.
