Peptic Ulcer Disease (PUD) remains a prevalent gastroenterological disorder, with significant morbidity and mortality. Despite advances in understanding its pathophysiology and the availability of effective treatments, the management of PUD continues to pose challenges. This article will discuss modern strategies for the effective management of PUD.
Helicobacter pylori infection and nonsteroidal anti-inflammatory drugs (NSAIDs) are the primary causes of PUD. Both disrupt the gastric mucosal defense and repair systems, leading to ulcer formation. Recent studies also suggest a genetic predisposition, further complicating the disease's pathophysiology.
Endoscopy with biopsy remains the gold standard for diagnosing PUD. However, non-invasive tests such as urea breath test, stool antigen test, and serology are increasingly used due to their convenience and cost-effectiveness.
The mainstay of PUD treatment is the eradication of H. pylori, achieved through a combination of proton pump inhibitors and antibiotics. The choice of antibiotics should be guided by local antibiotic resistance patterns. For NSAID-induced ulcers, discontinuation of the offending drug and use of proton pump inhibitors are recommended. Adjunctive therapies such as antacids and sucralfate can provide symptomatic relief but do not contribute to ulcer healing.
Emerging strategies include the use of probiotics to modulate the gut microbiota and the development of vaccines against H. pylori. Research is also underway to identify genetic markers for PUD to enable personalized treatment plans.
Effective management of PUD requires a comprehensive approach that includes understanding the pathophysiology, making an accurate diagnosis, and implementing appropriate treatment strategies. As we continue to decode PUD, it is hoped that new insights will lead to more effective and personalized treatments for this common and potentially serious condition.
