Abdominal pain, a common complaint in clinical practice, presents a unique diagnostic challenge due to its broad differential. This article aims to provide a comprehensive approach to evaluating abdominal pain, highlighting key aspects of history taking, physical examination, and investigative strategies.
A detailed history is paramount in narrowing the differential diagnosis. The onset, location, character, associated symptoms, and alleviating/aggravating factors of the pain should be thoroughly explored. It's also crucial to inquire about any relevant past medical, surgical, familial, or psychosocial history.
Physical examination should be systematic and comprehensive. Inspection, auscultation, palpation, and percussion should be performed in that order. Special attention should be paid to signs of peritonitis or surgical abdomen, which necessitate immediate surgical referral.
Laboratory investigations should include a complete blood count, renal function, liver function, and amylase/lipase levels. Imaging modalities should be selected based on the suspected diagnosis. For instance, ultrasound is the first-line for suspected gallbladder disease, while CT scan is preferred for suspected diverticulitis or appendicitis. Endoscopic procedures may be indicated based on the patient's age, risk factors, and the nature of the pain.
The management of abdominal pain is tailored to the underlying etiology. Analgesia should be provided, but with caution to not mask the signs of a surgical abdomen. Empiric treatment should be avoided unless the clinical scenario strongly suggests a specific diagnosis. Consultation with specialists should be sought when appropriate.
Evaluating abdominal pain requires a systematic approach, beginning with a thorough history and physical examination, followed by targeted investigations. It's crucial to maintain a broad differential and to reassess the patient frequently, as the clinical picture can evolve over time. By adopting such a comprehensive approach, clinicians can enhance their diagnostic accuracy, thereby ensuring optimal patient care.
