Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors represent a major breakthrough in lipid management. These medications, including alirocumab and evolocumab, have demonstrated efficacy in reducing low-density lipoprotein cholesterol (LDL-C) levels, even in patients with statin intolerance. Additionally, the advent of inclisiran, a small interfering RNA molecule, offers a promising new approach with its twice-yearly dosing regimen.
The American College of Cardiology (ACC) and the American Heart Association (AHA) released updated guidelines in 2019, emphasizing a more personalized approach to lipid management. The guidelines recommend high-intensity statin therapy for patients under 75 with atherosclerotic cardiovascular disease (ASCVD), and moderate-intensity therapy for those over 75 or with certain comorbidities.
