Peptic ulcer disease (PUD) continues to be a significant global health concern, with its management evolving over the years. This article aims to provide an update on the advancements in diagnosis and therapeutic approaches for PUD.
Diagnostic techniques for PUD have greatly improved. The traditional invasive methods, such as endoscopy and biopsy, are now supplemented with non-invasive techniques. Particularly, the Urea Breath Test (UBT) and Stool Antigen Test (SAT) have become the methods of choice for the diagnosis of Helicobacter pylori, the most common cause of PUD. These tests are accurate, cost-effective, and offer the advantage of monitoring the success of eradication therapy.
Therapeutic strategies for PUD have also seen significant advancements. The focus has shifted from acid suppression to eradication of H. pylori, with triple or quadruple therapy being the standard. However, increasing antibiotic resistance has led to the development of novel therapeutic strategies. These include sequential therapy, concomitant therapy, and hybrid therapy, which have shown higher eradication rates.
Additionally, the role of probiotics in PUD management is gaining attention. Several studies indicate that adjuvant use of certain probiotics can enhance the efficacy of H. pylori eradication therapy and reduce side effects. However, further research is needed to establish their routine use in clinical practice.
In conclusion, the diagnosis and treatment of PUD have undergone significant advancements, improving patient outcomes. The incorporation of non-invasive diagnostic methods and novel therapeutic strategies have optimised the management of PUD. However, the increasing antibiotic resistance and potential role of probiotics require further exploration to enhance the therapeutic approach for PUD.
