Unexplained weight loss (UWL), defined as a loss of more than 5% of total body weight within a six to twelve month period without a clear etiology, is a common yet challenging scenario in clinical practice. It can be an early sign of a serious underlying condition, necessitating a comprehensive and systematic approach to evaluation.
Initial evaluation should involve a thorough history and physical examination to identify potential clues. Key aspects include dietary habits, medication use, psychosocial factors, and accompanying symptoms. Laboratory investigations such as complete blood count, metabolic panel, thyroid function tests, and inflammatory markers may also be indicated.
The diagnostic approach should be guided by the patient's age, risk factors, and clinical presentation. In older adults, malignancy and non-malignant gastrointestinal diseases are common causes, while in younger patients, psychiatric conditions such as depression and eating disorders should be considered. If initial investigations are inconclusive, further diagnostic testing such as imaging or endoscopy may be warranted.
Management should be tailored to the underlying cause, if identified. In cases where no clear cause is found, a symptom management approach may be beneficial. This may involve dietary modifications, nutritional supplementation, and addressing any identified psychosocial issues. Regular follow-up is crucial to monitor weight and assess response to treatment.
Evaluating UWL requires a comprehensive and systematic approach, with careful consideration of patient characteristics, clinical presentation, and diagnostic findings. While it can be a challenging scenario, a structured approach can aid in identifying the underlying cause and guiding management. Continued research is needed to further refine diagnostic algorithms and management strategies for this common clinical conundrum.
