Chronic kidney disease (CKD) is a globally prevalent condition marked by progressive loss of renal function, with significant implications for morbidity, mortality, and healthcare resource utilization. Early detection and risk stratification remain pivotal in mitigating disease progression and associated complications, particularly cardiovascular events. Recent advances in understanding the pathophysiology of CKD have propelled the development of novel therapeutic interventions, including sodium-glucose co-transporter-2 (SGLT2) inhibitors, non-steroidal mineralocorticoid receptor antagonists (nsMRAs), and evolving biomarkers for earlier diagnosis. This review provides an in-depth analysis of CKD epidemiology, pathophysiology, risk factors, clinical features, diagnostic strategies, management approaches, and emerging therapies, emphasizing evidence-based recommendations for optimal patient care.
Chronic kidney disease constitutes a major global health concern, affecting approximately 10% of the world’s population and contributing to increased cardiovascular risk, decreased quality of life, and heightened healthcare expenditure. CKD is defined by a sustained reduction in glomerular filtration rate (GFR) and/or evidence of kidney damage, most commonly proteinuria, for a duration exceeding three months. The insidious nature of CKD progression underscores the necessity for early identification and intervention to prevent irreversible renal injury and to reduce the burden of end-stage kidney disease (ESKD). The integration of recent clinical guidelines and emerging therapeutic strategies is essential for the comprehensive management of CKD in contemporary clinical practice.
CKD has reached pandemic proportions, with recent data from the Global Burden of Disease Study estimating over 850 million people worldwide affected. Its prevalence escalates with advancing age and is particularly high among individuals with diabetes, hypertension, and cardiovascular disease. CKD is associated with a disproportionate impact on morbidity and mortality, accounting for significant years of life lost and disability-adjusted life years (DALYs). Notably, CKD is a principal driver of health disparities, with disadvantaged populations experiencing higher incidence, faster progression, and limited access to renal replacement therapies. The economic impact is considerable, stemming from increased hospitalizations, need for dialysis, and transplantation.
CKD arises from a constellation of pathogenic mechanisms that culminate in nephron loss and progressive renal dysfunction. Central to its pathogenesis are glomerular hyperfiltration, chronic inflammation, oxidative stress, activation of the renin-angiotensin-aldosterone system (RAAS), and maladaptive fibrosis. Persistent injury triggers tubular epithelial-mesenchymal transition and interstitial fibrosis, leading to irreversible nephron dropout. In diabetes, hyperglycemia induces mesangial expansion and podocyte injury, while hypertensive nephrosclerosis results from arteriolar thickening and ischemic glomerular collapse. The interplay between metabolic, hemodynamic, and immunologic factors underpins disease progression and the development of systemic complications such as anemia, mineral bone disorder, and cardiovascular disease.
Major risk factors for CKD include diabetes mellitus, hypertension, cardiovascular disease, obesity, family history of renal disease, and advanced age. Non-modifiable contributors such as genetic predisposition (APOL1 variants in African ancestry) and prior acute kidney injury (AKI) episodes further elevate risk. Socioeconomic determinants, including limited healthcare access and environmental exposures, also play a pivotal role. Modifiable risk factors such as poor glycemic control, uncontrolled blood pressure, smoking, and dyslipidemia are amenable to targeted interventions that may slow CKD progression and reduce associated morbidity.
CKD is often clinically silent in its early stages, with symptoms typically manifesting at advanced stages (GFR <30 mL/min/1.73 m²). Common presentations include fatigue, anorexia, pruritus, nocturia, and edema. Laboratory findings may reveal anemia, hyperphosphatemia, hypocalcemia, and metabolic acidosis. Proteinuria and hematuria are frequently detected on urinalysis. The insidious onset underscores the importance of routine screening in high-risk populations for timely diagnosis and intervention.
Diagnosis of CKD is established by documenting a reduced GFR (<60 mL/min/1.73 m²) and/or evidence of kidney damage (e.g., albuminuria >30 mg/g creatinine) persisting for at least three months. Assessment includes serum creatinine-based eGFR estimation, urinary albumin-to-creatinine ratio (uACR), and renal imaging to evaluate structural abnormalities. Recent advances in biomarker discovery, such as neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule-1 (KIM-1), hold promise for earlier detection and risk stratification. Differential diagnosis should exclude reversible causes such as volume depletion, obstructive uropathy, and drug-induced nephrotoxicity.
Optimal management of CKD centers on slowing progression, minimizing complications, and reducing cardiovascular risk. Fundamental strategies include stringent blood pressure control (target <130/80 mmHg), glycemic optimization in diabetes (HbA1c <7%), and RAAS blockade with angiotensin-converting enzyme inhibitors (ACEi) or angiotensin receptor blockers (ARBs). Lifestyle modifications, encompassing dietary protein and salt restriction, weight management, and smoking cessation, are universally recommended. Anemia, metabolic bone disease, and hyperkalemia should be monitored and addressed per current guidelines. Timely referral to nephrology is warranted for advanced CKD (stage 4/5), refractory complications, or rapid GFR decline.
Recent years have witnessed transformative advances in CKD therapeutics. SGLT2 inhibitors, initially developed for glycemic control, have demonstrated robust renoprotective and cardiovascular benefits in patients with and without diabetes, as evidenced by DAPA-CKD and EMPA-KIDNEY trials. Non-steroidal MRAs such as finerenone have shown efficacy in reducing albuminuria and delaying CKD progression in diabetic populations. Ongoing research into anti-fibrotic agents, endothelin receptor antagonists, and novel biomarkers continues to expand the therapeutic armamentarium. Precision medicine approaches and machine learning algorithms for risk prediction and individualized care represent promising frontiers in CKD management.
Contemporary guidelines from KDIGO and major nephrology societies endorse routine screening for CKD in high-risk populations, utilization of ACEi/ARBs for renoprotection, and integration of SGLT2 inhibitors in appropriate patients. Blood pressure and glycemic targets should be individualized, factoring in comorbidities and patient preferences. Shared decision-making, patient education, and multidisciplinary collaboration are emphasized to optimize outcomes and enhance quality of life. Early nephrology involvement is crucial for pre-ESKD planning and timely initiation of renal replacement therapy when indicated.
Chronic kidney disease remains a formidable challenge in contemporary medicine, necessitating a multifaceted approach encompassing early detection, risk reduction, and adoption of novel therapeutic modalities. Recent advances, particularly in SGLT2 inhibition and non-steroidal MRAs, have revolutionized the management landscape, offering hope for improved renal and cardiovascular outcomes. Continued research, guideline refinement, and health system innovations are essential to address the global CKD burden and achieve optimal patient-centered care.
