Anemia is classified based on the mean corpuscular volume (MCV) into microcytic, normocytic, or macrocytic anemia. Microcytic anemia often indicates iron deficiency or thalassemia. Normocytic anemia could suggest chronic disease, acute blood loss, or hemolysis. Macrocytic anemia is usually a result of vitamin B12 or folate deficiency, myelodysplastic syndrome, or the effect of certain drugs.
Further investigations like iron studies, vitamin B12 and folate levels, hemoglobin electrophoresis, or bone marrow examination are guided by the MCV classification and clinical suspicion. Special considerations are necessary in certain populations. For example, in pregnant women, physiological changes may mimic anemia. In the elderly, anemia could be an early sign of a serious underlying condition like malignancy. Tailoring the diagnostic approach to the patient's unique context is crucial.
