Abstract

Emerging scientific interest in integrative medicine has prompted a reevaluation of traditional pathophysiological models, among which the Ayurvedic concept of dosha dysregulation is gaining attention for its potential explanatory value in chronic disease evolution. This review critically examines the clinical and molecular cascades triggered by dosha imbalances, synthesizing contemporary research, guideline-based recommendations, and mechanistic insights relevant to modern chronic disease management. We discuss epidemiological patterns, risk factors, pathophysiological underpinnings, clinical presentations, diagnostic challenges, and therapeutic strategies, integrating recent advances and guideline recommendations to provide a comprehensive resource for healthcare professionals.

Introduction

Chronic diseases such as diabetes mellitus, cardiovascular disorders, and autoimmune syndromes continue to be leading causes of morbidity and mortality worldwide. Despite molecular advances, the multifactorial etiology and complex progression of these diseases often elude reductionist biomedical models. Ayurveda, the ancient Indian system of medicine, attributes the genesis and progression of chronic illnesses to dysregulation of fundamental physiological energies known as doshas Vata, Pitta, and Kapha. This review explores the evolving scientific discourse on dosha dysregulation cascades, with an emphasis on mechanistic and clinical implications for chronic disease evolution.

While subjective, these approaches are being refined with emerging biomarker studies and digital phenotyping. Integration of conventional diagnostic modalities (e.g., metabolic panels, imaging, inflammatory markers) with dosha-based profiling holds promise for early detection and personalized risk assessment. However, the lack of universally accepted standards for dosha evaluation remains a significant barrier to widespread clinical adoption.

Treatment & Management

Management strategies focus on restoring doshic equilibrium through individualized interventions. Collaboration between Ayurvedic practitioners and conventional healthcare providers is advocated to ensure safety and optimize clinical outcomes. Ongoing research and continuous professional education are essential for the evidence-based evolution of these guidelines.
