Gastroesophageal reflux disease (GERD) is a chronic condition affecting millions worldwide. Despite its prevalence, the complex pathophysiology often leads to difficulties in diagnosis and treatment. This article aims to provide an overview of the current understanding of GERD, its diagnostic procedures, and advanced treatment strategies.
GERD is characterized by the frequent occurrence of acid reflux, where stomach acid frequently flows back into the tube connecting the mouth and stomach (esophagus). This backwash (acid reflux) can irritate the lining of the esophagus, causing symptoms such as heartburn and regurgitation. The pathophysiology of GERD is multifactorial, involving factors like lower esophageal sphincter abnormalities, esophageal motility disorders, and gastric abnormalities.
Diagnosis of GERD typically involves a combination of symptom assessment, endoscopy, and pH monitoring. While symptom assessment can be useful, it often lacks specificity. Endoscopy allows for the direct visualization of the esophagus and stomach, aiding in the identification of esophagitis or Barrett’s esophagus. pH monitoring, either through 24-hour esophageal pH monitoring or wireless pH monitoring, provides objective evidence of acid reflux, helping confirm the diagnosis.
While lifestyle modifications and proton pump inhibitors (PPIs) remain first-line treatments, advanced therapeutic options are available for those who do not respond adequately. These include endoscopic treatments like radiofrequency ablation, transoral incisionless fundoplication, and anti-reflux mucosectomy. Surgical interventions, such as laparoscopic Nissen fundoplication, may also be considered in refractory cases.
GERD is a complex disease that requires a comprehensive approach to diagnosis and treatment. Understanding its pathophysiology, coupled with advances in diagnostic procedures and treatment strategies, can significantly improve patient outcomes. As our understanding of GERD continues to evolve, so too will our ability to effectively diagnose and manage this common condition.
