The landscape of peptic ulcer disease (PUD) management has dramatically evolved over the past few decades, thanks to the advancement in diagnostic tools, pharmacotherapy, and endoscopic techniques. This article seeks to provide an update on the latest advancements in PUD management.
Modern diagnostic techniques for PUD have significantly improved. The development of high-definition endoscopy and narrow-band imaging has enhanced the detection and characterization of peptic ulcers. Moreover, the implementation of non-invasive tests for Helicobacter pylori has simplified the diagnostic process.
Pharmacotherapy remains a cornerstone in PUD management. The advent of proton pump inhibitors (PPIs) has revolutionized the treatment of PUD, providing superior acid suppression and promoting ulcer healing. Recent advancements include the introduction of vonoprazan, a potassium-competitive acid blocker, which has shown to be more effective than traditional PPIs in eradicating H. pylori and healing gastric ulcers.
Endoscopic techniques have greatly advanced in managing complicated PUD. Endoscopic hemostasis, including injection therapy, mechanical clips, and thermal coagulation, is now the standard of care for bleeding ulcers. Furthermore, endoscopic submucosal dissection (ESD) has emerged as a promising technique for early gastric cancer associated with PUD.
Eradication of H. pylori plays a crucial role in PUD management. The development of quadruple therapy, including bismuth, has improved the eradication rates. The recent introduction of high-dose dual therapy with amoxicillin and PPIs provides an effective alternative with fewer side effects.
In conclusion, the management of PUD has significantly advanced, offering better diagnostic accuracy, more effective pharmacotherapy, and improved endoscopic techniques. Continuous research and innovation in this field promise further improvements in patient outcomes.
