Elevated liver enzymes often present a diagnostic challenge for clinicians due to the myriad of potential underlying etiologies. This article aims to provide a comprehensive approach to the diagnosis and management of this common clinical issue.
Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) are the most commonly assessed liver enzymes. An isolated elevation in ALT or AST may be indicative of hepatocellular injury, while an increased alkaline phosphatase (ALP) may suggest cholestasis. Gamma-glutamyl transpeptidase (GGT) can help differentiate hepatic from bone disease when ALP is elevated. Bilirubin levels can also provide useful diagnostic information.
Common causes include alcohol consumption, viral hepatitis, nonalcoholic fatty liver disease (NAFLD), and medication-induced hepatotoxicity. Less common causes include autoimmune hepatitis, hemochromatosis, Wilson's disease, and alpha-1 antitrypsin deficiency.
History and physical examination are fundamental. Alcohol consumption, medication history, and risk factors for viral hepatitis should be assessed. Laboratory investigations should include a complete blood count, coagulation studies, and serologic tests for viral hepatitis. Imaging studies can also be helpful.
Management depends on the underlying cause. Alcohol cessation, weight loss, and discontinuation of hepatotoxic medications are often beneficial. Antiviral therapy is indicated for viral hepatitis. In some cases, referral to a hepatologist may be necessary.
Elevated liver enzymes are a common clinical issue that requires a thorough and systematic approach to diagnosis and management. By understanding the interpretation of liver enzyme tests and being aware of the common causes, clinicians can provide effective patient care, ultimately improving patient outcomes.
