Pyrexia, commonly known as fever, is a prevalent clinical symptom indicating an underlying pathophysiological process. It is often a protective response to various stimuli, including infections, malignancies, or immune disorders. Understanding its mechanisms, diagnostic approaches, and management strategies is crucial for all healthcare professionals.
Pyrexia results from a complex interplay of cytokines, prostaglandins, and the hypothalamus. Pyrogens, substances that can induce fever, trigger the release of cytokines such as interleukin-1 and tumour necrosis factor. These cytokines stimulate the production of prostaglandin E2 (PGE2) in the hypothalamus, resetting the body's 'thermostat' to a higher level, leading to fever.
A comprehensive history and physical examination form the cornerstone of pyrexia diagnosis. It is essential to identify potential sources of infection, exposure history, travel history, and associated symptoms. Laboratory investigations, including complete blood count, blood cultures, and imaging studies, may be required depending on the clinical context.
Management of pyrexia involves treating the underlying cause and symptomatic relief. Antipyretics, such as paracetamol and non-steroidal anti-inflammatory drugs (NSAIDs), are commonly used for symptom control. However, the decision to treat fever should be individualized, considering the patient's comfort and the potential benefits of fever in fighting infections.
In cases where infection is the identified cause of fever, appropriate antimicrobial therapy is crucial. Empirical therapy should be initiated promptly in septic patients, followed by targeted therapy based on culture and sensitivity results.
Pyrexia is a complex clinical symptom with diverse etiologies. A systematic approach to diagnosis and management, understanding the role of fever in the host defense mechanism, and judicious use of antipyretics can optimize patient outcomes. Continuous research is required to further our understanding of pyrexia and improve its management.
