Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant health concern worldwide. This article provides an updated approach to the diagnosis and treatment of enteric fever, incorporating recent advancements in medical science.
Early diagnosis of enteric fever is crucial for effective management. The gold standard for diagnosis remains blood culture, which has a sensitivity of 40-60%. However, this method is limited by the need for sophisticated laboratories and slow turnaround times. Recent advancements include the introduction of rapid diagnostic tests (RDTs) such as the Typhidot and Tubex TF, which are based on detecting IgM antibodies against the bacteria. These RDTs have shown promising results in endemic regions, with higher sensitivity and specificity compared to traditional methods.
Treatment of enteric fever has been complicated by the emergence of multidrug-resistant strains. The World Health Organization currently recommends ceftriaxone or cefixime for uncomplicated cases, and ciprofloxacin for complicated cases. However, due to increasing resistance, azithromycin has emerged as a viable alternative. Adjunctive therapies, such as probiotics and zinc supplementation, are also being explored to reduce the duration and severity of illness.
Prevention strategies are key in controlling the spread of enteric fever. Vaccination remains the most effective measure, with the Vi polysaccharide and Ty21a vaccines showing good efficacy. In addition, improving water quality, sanitation, and hygiene practices are crucial in endemic regions to reduce transmission.
Enteric fever continues to present significant challenges in diagnosis and treatment. Incorporating rapid diagnostic tests into clinical practice and exploring alternative treatment options, such as azithromycin, may improve patient outcomes. A multifaceted approach encompassing prevention strategies, including vaccination and improved sanitation, is essential to control the spread of this disease.
