Peptic ulcer disease (PUD) is a prevalent gastrointestinal disorder characterized by the formation of sores in the lining of the stomach, upper small intestine, or lower esophagus. Over the past few decades, the understanding of PUD pathophysiology has significantly improved, leading to the development of advanced diagnostic tools and therapeutic strategies. This article aims to provide an overview of the recent advancements in PUD management, thereby improving patient outcomes and reducing healthcare costs.
Helicobacter pylori infection and nonsteroidal anti-inflammatory drug (NSAID) use are the two primary causes of PUD. The pathogenesis involves an imbalance between the gastroprotective (mucus and bicarbonate secretion, prostaglandin synthesis) and aggressive factors (acid and pepsin secretion). Recent research has highlighted the role of genetic susceptibility, stress, and other lifestyle factors in PUD development.
Accurate diagnosis of PUD is crucial for effective management. Traditional diagnostic methods such as endoscopy and barium contrast x-rays continue to be useful. However, newer diagnostic tools offer advantages in terms of accuracy, convenience, and cost-effectiveness. The urea breath test, stool antigen test, and serologic testing are non-invasive tests for H. pylori detection. Endoscopic ultrasound and confocal laser endomicroscopy allow for detailed examination of the ulcer and surrounding tissues. Molecular methods such as polymerase chain reaction (PCR) and next-generation sequencing help in identifying antibiotic-resistant H. pylori strains, thereby guiding therapy.
With a better understanding of PUD pathophysiology, treatment strategies have evolved significantly. The primary aim of treatment is to eliminate the cause, heal the ulcer, and prevent complications. H. pylori eradication therapy, involving a combination of proton pump inhibitors (PPIs) and antibiotics, is the first line of treatment. For NSAID-induced ulcers, discontinuation of NSAID or switching to a lower-risk alternative along with PPI therapy is recommended. Recent advancements include the use of novel agents like vonoprazan (a potassium-competitive acid blocker), rebamipide (a mucosal protective agent), and gastroretentive drug delivery systems for sustained drug release.
Complications such as bleeding, perforation, and gastric outlet obstruction significantly increase the morbidity and mortality associated with PUD. Endoscopic hemostasis techniques like injection, thermal coagulation, and mechanical methods have revolutionized the management of bleeding ulcers. Laparoscopic and endoscopic techniques have reduced the need for open surgery in managing perforation and obstruction. Prophylactic use of PPIs and H2 receptor antagonists has been effective in preventing NSAID-associated ulcers.
Despite advancements in diagnostic and therapeutic strategies, the role of patient education and lifestyle modification cannot be undermined. Educating patients about the harmful effects of smoking, alcohol, and stress; the importance of medication adherence; and the need for regular follow-up can significantly impact disease management. Dietary modifications and regular physical activity also contribute to ulcer healing and prevention.
Advancements in the understanding of PUD pathophysiology, diagnostic tools, and treatment strategies have significantly improved patient outcomes. However, the management of PUD remains a challenge due to the high recurrence rate and potential complications. Future research should focus on developing personalized treatment strategies based on genetic susceptibility, H. pylori strain type, and individual risk factors. The role of patient education and lifestyle modification in disease management should also be emphasized. As healthcare professionals, staying updated with these advancements will enable us to provide optimal care to our patients.
