Peripheral neuropathy typically presents with symptoms such as numbness, tingling, pain, and weakness, predominantly in the hands and feet. A careful history and physical examination, focusing on the pattern of symptom onset, distribution, progression, and associated systemic symptoms, are vital in the initial evaluation.
After the clinical evaluation, laboratory and imaging studies can help identify the etiology. Basic lab tests including complete blood count, metabolic panel, and vitamin B12 levels should be considered. Depending on the suspected cause, more specific tests such as genetic testing, autoimmune panels, or infectious disease tests may be needed. Nerve conduction studies and electromyography can help confirm the diagnosis and provide information about the severity and distribution of nerve damage.
In certain cases, a nerve or skin biopsy may be useful. A nerve biopsy can reveal axonal loss, demyelination, or vasculitis, while a skin punch biopsy can show loss of intraepidermal nerve fibers, indicative of small fiber neuropathy.
While diabetes is the most common cause of peripheral neuropathy, clinicians should consider less common etiologies, especially when the presentation is atypical or the patient does not respond to initial treatment. These include hereditary neuropathies, toxins, medications, and malignancies.
Evaluating peripheral neuropathy requires a systematic approach that includes a thorough clinical evaluation, appropriate laboratory and imaging studies, and consideration of both common and rare causes. By navigating this diagnostic maze, clinicians can identify the underlying cause and initiate appropriate treatment, improving outcomes for patients with this challenging condition.
