Acute Coronary Syndrome (ACS) remains a significant cause of morbidity and mortality worldwide. Despite major advances in diagnosis and treatment, optimizing patient outcomes continues to be a challenge. This article aims to provide an updated approach to ACS management, focusing on early identification, risk stratification, and appropriate therapeutic interventions.
Timely diagnosis of ACS is crucial. Recent advancements in biomarkers, specifically high-sensitivity cardiac troponins, have improved early detection. Risk stratification, using tools such as the GRACE score, aids in identifying high-risk patients who may benefit from aggressive interventions.
Antiplatelet therapy remains the cornerstone of ACS management. Dual antiplatelet therapy (DAPT) with aspirin and a P2Y12 inhibitor is recommended. The choice of P2Y12 inhibitor should be individualized based on patient risk factors, potential drug-drug interactions, and bleeding risk. The role of novel anticoagulants is being explored, with some demonstrating promising results in reducing recurrent ischemic events.
Early invasive strategy is recommended for high-risk patients. Percutaneous coronary intervention (PCI) has been shown to improve outcomes in these patients. The timing of PCI should be individualized, with immediate intervention necessary in patients with ongoing ischemia or hemodynamic instability. For stable patients, a delayed intervention approach may be considered.
Secondary prevention strategies including lifestyle modifications, control of cardiovascular risk factors, and long-term use of medications such as statins, beta-blockers, and ACE inhibitors are essential for improving long-term outcomes.
Optimizing patient outcomes in ACS requires a comprehensive approach that includes early identification, risk stratification, appropriate pharmacologic management, an individualized invasive strategy, and aggressive secondary prevention. Continuous advancements in this field necessitate an updated understanding of best practices to ensure optimal patient care.
