Anemia, a condition characterized by a decrease in the total amount of red blood cells (RBCs) or hemoglobin in the blood, presents a complex diagnostic challenge for healthcare professionals due to its multifactorial etiology. This article aims to provide a comprehensive guide to the diagnostic approach to anemia.
Anemia can be classified based on the size of RBCs (microcytic, normocytic, or macrocytic), or based on the etiology (nutritional, hemolytic, aplastic, or anemia of chronic disease). This initial classification can guide further diagnostic testing.
The first step in diagnosing anemia is a complete blood count (CBC) to assess RBC count, hemoglobin levels, and mean corpuscular volume (MCV). If anemia is confirmed, reticulocyte count, peripheral smear, and other specific tests such as iron studies or vitamin B12 levels can help determine the cause.
Nutritional anemias, such as iron deficiency anemia and vitamin B12 deficiency anemia, can be confirmed with serum ferritin and serum B12 measurements, respectively. A trial of supplementation can also be diagnostic.
Hemolytic anemia can be suspected from a high reticulocyte count and confirmed with tests like direct Coombs test. Aplastic anemia, a bone marrow failure syndrome, requires bone marrow biopsy for confirmation.
Anemia of chronic disease, now also called anemia of inflammation, can be diagnosed when anemia occurs in the setting of chronic illnesses like autoimmune diseases, infections, or malignancies, and other causes have been ruled out.
Understanding the complexity of anemia and its various types is crucial for healthcare professionals. A systematic approach to diagnosis, based on initial classification and targeted testing, can help identify the underlying cause and guide appropriate treatment.
