Understanding the pathophysiology of anemia is crucial in its diagnosis. Anemia can result from three primary processes: reduced erythropoiesis, increased destruction of red blood cells, or blood loss. Each of these processes presents with distinct clinical and laboratory features, aiding in the differential diagnosis.
The initial diagnostic approach involves a complete blood count (CBC) and a review of the peripheral smear. A low hemoglobin level confirms anemia, while the mean corpuscular volume (MCV) helps classify it as microcytic, normocytic, or macrocytic. The peripheral smear can provide additional clues about the etiology. Further investigations are guided by these initial findings.
Interpreting the results requires a systematic approach. Microcytic anemia is often due to iron deficiency or thalassemia. Normocytic anemia may indicate chronic disease, renal disease, or early iron deficiency. Macrocytic anemia can be caused by vitamin B12 or folate deficiency, myelodysplastic syndrome, or certain medications. The reticulocyte count, serum iron studies, and other specific tests can help confirm the diagnosis.
