Elevated liver enzymes are a common and perplexing issue encountered in clinical practice. They may indicate a wide range of hepatocellular injuries, from benign transient elevations to severe liver diseases. Liver enzyme elevations are often discovered incidentally during routine blood tests. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) are the most commonly evaluated enzymes. While ALT is more specific to the liver, AST can also be elevated in conditions affecting the heart, muscles, and kidneys. Alkaline phosphatase (ALP) and gamma-glutamyl transferase (GGT) are indicative of cholestasis.
The first step in evaluating elevated liver enzymes is to confirm the abnormality with repeat testing. History taking and physical examination are crucial, focusing on risk factors for liver diseases, alcohol consumption, medication use, and symptoms of chronic liver disease. Initial laboratory investigations should include a complete blood count, coagulation profile, and serologic tests for viral hepatitis. Depending on the findings, further investigations may include imaging studies or liver biopsy.
Management is guided by the underlying cause. Lifestyle modifications are recommended in cases of non-alcoholic fatty liver disease. Viral hepatitis requires antiviral therapy, while autoimmune liver diseases necessitate immunosuppressive treatment. In some cases, no specific treatment is required, and the patient can be reassured with regular monitoring.
Decoding the diagnostic dilemma of elevated liver enzymes requires a systematic approach. By understanding the possible causes, conducting a thorough clinical assessment, and ordering appropriate investigations, clinicians can accurately diagnose and manage the underlying liver condition. This comprehensive approach ensures optimal patient outcomes and contributes to the advancement of hepatology practice.
