Liver cirrhosis, a chronic and progressive disease, is characterized by fibrosis and the formation of regenerative nodules, leading to a disruption of the normal architecture of the liver. This article aims to provide a comprehensive overview of the complications associated with liver cirrhosis and their management.
Portal hypertension, a common complication, results from increased resistance to portal blood flow. It leads to complications such as variceal bleeding, ascites, and hepatic encephalopathy. Management includes non-selective beta-blockers, endoscopic band ligation, and transjugular intrahepatic portosystemic shunt (TIPS).
Ascites, the most common complication of cirrhosis, is managed with a low-sodium diet and diuretics. Refractory cases may require therapeutic paracentesis, TIPS, or liver transplantation.
Hepatic encephalopathy, characterized by cognitive impairment, is managed with lactulose and rifaximin. Patient education is critical to ensure adherence to treatment and to avoid precipitating factors.
Hepatorenal syndrome, a severe complication, is managed with vasoconstrictors and albumin. In refractory cases, renal replacement therapy or liver transplantation may be considered.
Hepatocellular carcinoma, a long-term complication of cirrhosis, requires surveillance. Management includes resection, liver transplantation, and locoregional therapies.
Understanding and managing the complications associated with liver cirrhosis is critical for improving patient outcomes. Early detection, appropriate intervention, and patient education play a key role in the management of these complications. As healthcare professionals, we must stay abreast of the latest advancements in the field to provide the best care to our patients.
