Chronic cough, defined as a cough persisting for more than eight weeks, is a common clinical conundrum faced by healthcare professionals. Despite its high prevalence, the underlying etiology often remains elusive, leading to diagnostic challenges and treatment delays.
Chronic cough is a complex, multi-faceted symptom, often resulting from an interplay of various pathophysiological processes. The most common causes include gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and asthma. However, a significant subset of patients has non-specific or idiopathic chronic cough, further complicating the diagnostic approach.
A comprehensive, systematic approach is essential in diagnosing chronic cough. A thorough clinical history and physical examination should be the first step, followed by targeted investigations based on clinical suspicion. These may include spirometry, chest radiography, and upper gastrointestinal endoscopy. In cases where the etiology remains unclear, more advanced diagnostic modalities such as bronchoscopy or high-resolution computed tomography may be warranted.
Management of chronic cough should be tailored to the underlying cause. In cases of GERD, UACS, or asthma, appropriate management of these conditions often leads to resolution of the cough. For patients with idiopathic chronic cough, a trial of empirical therapy with antitussive agents may be considered. In refractory cases, referral to a specialist or a multidisciplinary team may be necessary.
Chronic cough is a common yet challenging condition that requires a comprehensive, systematic approach for effective management. By understanding the complex pathophysiology and adopting a targeted diagnostic and therapeutic approach, healthcare professionals can significantly improve the quality of life of patients suffering from this debilitating symptom.
